Students that have been notified of failing to meet Satisfactory Academic Progress (SAP) are able to appeal the status by filling out this form. Students will need to meet with an academic advisor, but must first fill out the form and send it to chpsundergrad@ucf.edu . Once the form has been received, they can schedule an in-person or virtual appointment by calling 407-823-0010. Please allow 24-48 hours between form submission and appointment.
